Accident Insurance (Policy Series WS-ACC 8/08)
- 24 hour (and off-the-job coverage options)
- Indemnity amounts payable for many triggers such as:
- Hospitalization
- Emergency room
- AD&D
- Transportation
- Follow-up care
- Burns, lacerations, fractures, dislocations
- Optional Riders for Hospital confinement due to Sickness, an Enhanced Emergency Room Benefit & Physician Office/Urgent Care Treatment Benefit and a Wellness Benefit.
- Guarantee issue on the base plan for eligible employees, spouse and children
- Policy is portable should the employee leave his/her employer
Critical Illness Insurance (Policy Series WS-CI 4/12)
- Pays 100% of the benefit amount for the diagnosis of many covered conditions. Includes Cancer, Heart Attack, Stroke, End Stage Renal Disease, Major Organ Transplant, Coma, Paralysis, Severe Burns, Loss of Sight, Speech or Hearing, Alzheimer’s Disease, Amyotrophic Lateral Sclerosis (ALS) and Benign Brain Tumor.
- Pays 30% of the benefit amount for Coronary Artery Bypass, Angioplasty and Stent Insertion and Carcinoma In Situ.
- Children are covered to age 26 regardless of marital or dependency status – a 25% benefit is automatically included at no additional charge.
- Children have additional coverage for a new diagnosis of childhood conditions – Cerebral Palsy, Cystic Fibrosis, Down Syndrome, Spina Bifida and Cleft Palate/Lip.
- Includes a one-time Skin Cancer benefit of $300.
- Cancer benefit can be carved out to work with other coverages in place.
- No lifetime maximum – includes benefits for reoccurrences and additional occurrences.
